• Orders will be delivered to the hospital 3rd week of June; or $20 for home delivery.
• Tshirts will have printed front logo and printed backs INFORMATION SERVICES / SERVICES INFORMATIQUES.
• Crewnecks, 1/4 Zips and Hoodies include embroidered logo and printed back with INFORMATION SERVICES / SERVICES INFORMATIQUES.
  
• Jackets will have embroidered logo; backs are optional.

• EMBROIDERY REQUESTS THAT ARE NOT PAID FOR WILL NOT BE DONE.

Optional customizing must be purchased in addition:

• Add name embroidered to sleeve of jacket or sweaters
• Add department embroidered to back of jacket.
• Choose the "Customize" product you want and add it, as well as the jacket of your choice, to your cart.  Both items must be purchased for the optional customizing.
